Isabella opened it’s admission criteria to primary mental health in 2003, providing care to the chronically ill to prevent unnecessary hospitalization and maximizing safety and independence for those we care for.
Opening available for a full time: PSYCHIATRIC SENIOR NURSE
Performs an evaluation assessment visit to referred patients with primary psychiatric diagnoses at their home and/or facility to determine eligibility for the program and evaluates the home situation for safety. Assess patient's status and reports finding to primary physician/psychiatrist as well as home care interdisciplinary team. Working with the primary care nurse, the psychiatric senior nurse provides leadership in care plan development and instructs care team in appropriate intervention strategies with the client participation.
REQUIREMENTS & QUALIFICATIONS:
Current New York State license as a Registered Nurse with a Master’s degree preferred with a specialty in psychiatric or mental health nursing; must have nursing experience in an acute treatment unit of a psychiatric hospital, psychiatric home care, partial psychiatric hospitalization program or other outpatient psychiatric services. Demonstrated bi-lingual (English/Spanish) proficiency is preferred
