Physical Therapist (Crown Point IN USA)

1. Must have graduated from an accredited school and current licensure in ANY STATE as a Physical Therapist (PT).
2. Must have current CPR certification.
3. Develop a plan of care for each physical therapy patient.
4. Provide skilled physical therapy services/interventions in accordance with physician orders.
5. Assesses patient needs, plans for, evaluates, and modifies care to meet goals of physical therapy interventions.
6. Preferred Out Patient PT experience
