CLINICAL SUPERVISOR - AIDS SERVICES (san jose downtown, USA)

The Health Trust provides services to Santa Clara County residents with HIV/AIDS and empowers patients and families to solve their day to day needs.

Title: Clinical Supervisor, AIDS Services
Days/Hours: Full Time – Monday through Friday

Summary
The Clinical Supervisor, AIDS Services is responsible for overseeing and coordinating all services and the activities of medical case management and benefits counseling staff; is the direct supervisor of the Nurse Case Managers, Medical Social Workers and Benefits Specialists.

Requirements
• Previous experience in a supervisory position, preferably in a public health or community-based health care or social service organization.
• Previous experience in case management preferred.
• Master’s Degree required in Social Work, Counseling, or Psychology or other related field with similar qualifications, or licensed by the State of California as a Licensed Clinical Social Worker (LCSW), Marriage and Family Therapist (MFT), or Psychologist with at least two years professional experience working as part of an interdisciplinary team in a healthcare setting preferred
OR
• Registered Nurse (RN) licensed by the State of California who has two years experience as an RN, with at least one year in community nursing or public health preferred.
• Bachelor of Science degree in Nursing (BSN), or Public Health Nurse certificate (PHN) preferred.
• Knowledge of and ability to use Microsoft Office products.
• Employment background screening required.
• Excellent oral and written communication and interpersonal skills to perform essential duties, provide effective leadership, communicate effectively with diverse populations, develop and maintain a wide variety of internal and external working relationships and to maintain a high standard of professional conduct.
• Able to work independently, manage time effectively, prioritize, problem solve, and exercise critical thinking and good judgment with minimal supervision.
• Able to work as a team player, utilize and offer interdisciplinary collaboration and support.
• Able to efficiently handle demanding workload involving multiple tasks, and master detailed and complex information, and coordination of care
• Able to relate to other people, handle conflict, crisis situations, and difficult clients
• Ability to build collaborative relationships in the delivery of services.
• Strong clinical skills
• Knowledge of case management preferred.
• Knowledge of HIV/AIDS, related issues, and concerns
• Knowledge of chronic and acute disease and how it impacts patient and family functioning
• Knowledge of substance use and dependence, assessment, and intervention strategies
• Knowledge of the Harm/risk Reduction model and Behavior Change theory
• Knowledge of mental health issues, assessment, and intervention strategies
• Knowledge and utilization of community resources
• Knowledge and utilization of Public and private benefit and entitlement programs (e.g. Social Security, Medi-Cal, Medicare, State Disability, General Assistance, IHSS, etc.)
• Knowledge of child, elder, and dependent adult abuse and domestic violence reporting requirements and other significant regulations affecting practice
• Knowledge of and ability to use MS Office products.
• Must pass employment screening.
